A free, new event to learn about overcoming barriers to health and taking control of your care, presented by the Wandsworth Self-Management Service and Wandsworth Self Help Programme. All welcome - from patients and carers, to GPs, health and support workers, community and voluntary sector groups and anyone else interested in self-management and self-help.

Self Management is about enabling and empowering people living with long term health conditions, or carers, to experience, learn, and become proficient at using skills and techniques to take control of their condition/caring situation, while meeting others and sharing their experience and expertise through peer-led education, training or support.

Wandsworth Self Management Service (WSMS) provides access to a variety of options for people living with long term conditions and their carers. These include awareness sessions, opportunities to join or set up groups, access to information on the internet and self management programmes like the Expert Patients Programme.

Wandsworth Self Help Programme is a new project run by Lifetimes Charity, offering training and support to empower people with a shared condition or life situation such as addiction, loss, or survival to join together. The mutual aim of a self help group is to give as well as to receive help, learn and share information and practical ways of managing with their shared challenge. In doing so members inspire each other, gain more control and feel empowered to cope with their challenges and experiences.

This event will cover:

  • Introductions to the Wandsworth Self Management Service, Wandsworth Self Help Programme and Care4Me Community Directory
  • The health journey, effective listening and assumptions we all make
  • Getting the best of your conversations with your healthcare professionals
  • Finding solutions - how to solve problems and challenges
  • Getting past uncertainty
  • Overcoming barriers, using importance and confidence skills to move forward
  • Creating and sharing a plan
  • Self Management, Expert Patients Programme and Condition Specific courses available in Wandsworth
  • How to access information in Wandsworth
  • How to join, set up or find out more about Self Help Groups in Wandsworth

Event Agenda:
The day has a packed agenda from 10.00am to 4.30pm (with breaks).
You will also be able to browse the information stalls and find suitable resources during the day.
Lunch and Refreshments will be provided.

How to get to York Gardens Library and Community Centre:
The Library and Community Centre is located inside York Gardens. Buses 44 and 170 stop on the East side of the Gardens, on York Road, or alternatively it's a 10-15 minute walk from Clapham Junction Station (Grant Road Exit) through the Winstanley Estate. See a map here:
